Does the Apple Watch Measure Blood Pressure?

No. There is no Apple Watch model, and no app, that will show you a systolic and diastolic reading taken by the watch itself. No amount of updating changes that — the hardware has no cuff, and pressure cannot be read from an optical sensor alone.
What the watch does have, since watchOS 26, is hypertension notifications: a passive screening feature that watches for a pattern over 30 days and tells you to go get a cuff. That is a smaller promise and a more honest one, and this guide explains exactly what it can and cannot tell you.
What hypertension notifications actually are
The watch's optical heart sensor already shines light into your wrist to track your pulse. Apple's algorithm looks at something subtler in that signal: how your blood vessels respond to each heartbeat. Over a rolling 30-day window it accumulates enough data to judge whether that response is consistent with chronic high blood pressure — and if it is, it notifies you.
The details worth knowing:
- It runs in the background. There is nothing to start, no measurement to take, no reading to hold still for.
- It takes about 30 days. A notification is a verdict on a month of data, not on today.
- It never produces a number. You get a notification or you get nothing.
- It was cleared by the FDA in September 2025 and launched across more than 150 countries and regions.
- It was trained and validated at scale — training data from studies totalling over 100,000 participants, and a clinical validation study of over 2,000.
Apple's own projection when launching it was that it would flag roughly a million people with undiagnosed hypertension in its first year. That is the point of the feature: high blood pressure has almost no symptoms, so the people most likely to benefit are the ones with no reason to check.
Which models, and who it is for
Supported models: Apple Watch Series 9 and later, and Apple Watch Ultra 2 and later, running watchOS 26 with a compatible iPhone. Older watches do not get the feature — the requirement is the hardware, not just the software.
Not intended for:
- Anyone under 22
- Anyone already diagnosed with hypertension — if you have a diagnosis, the screening question is already answered, and your job is monitoring, not detection
- Anyone who is pregnant — blood pressure in pregnancy needs a different and closer kind of attention, including for conditions like pre-eclampsia, and this feature is not designed for it
You enable it in the Health app on iPhone, under the heart section. It needs the watch worn regularly, including during sleep for best results, since the algorithm wants a broad sample of your days.
What a notification means — and what it doesn't
If you get one: it means your data over the past month showed signs consistent with hypertension. It is not a diagnosis. Hypertension is diagnosed on actual measured pressures, by a clinician, usually across more than one occasion.
The right next step is the one Apple itself recommends: measure with a real cuff for seven days, morning and evening, two readings a minute apart each time — the standard home monitoring protocol. Then take that log to your doctor. A week of cuff readings is a far stronger basis for a conversation than the notification alone.
If you never get one: that is not a clean bill of health, and this is the part most easily misread. Apple has been explicit that the feature does not detect all cases of hypertension. It is tuned to avoid false alarms, which necessarily means missing some real cases. If you have risk factors — family history, higher weight, high salt intake, diabetes, kidney disease, or you are simply over 40 — you should still be checking your blood pressure with a cuff on a schedule, notification or not.
The ECG app is a different thing entirely
The Apple Watch's ECG app is genuinely a measurement, and it gets confused with blood pressure constantly. An ECG records the electrical activity of your heart — the rhythm — and the watch's version is designed to check for atrial fibrillation. Blood pressure is the mechanical force of blood against artery walls. Nothing in an electrical trace gives you that number.

The same applies to heart rate. A low resting heart rate does not mean low blood pressure, and a racing pulse does not mean high blood pressure — the two move independently more often than people expect.
Using the Apple Watch alongside a real monitor
The combination is genuinely good, once you stop asking the watch to be a cuff.
- The watch handles detection and context. Hypertension notifications, high and low heart rate alerts, AFib history, plus sleep and activity data that explain why a week's readings look different.
- The cuff handles the numbers. Everything your doctor acts on — the categories, the treatment thresholds, the response to a medication change — is defined on cuff measurements.
- The Health app is the meeting point. Your cuff readings, however you record them, sit in the same place as the watch's data, so you can see them against each other.

Will a future Apple Watch measure blood pressure properly?
Apple has not announced one. Doing it without a cuff means solving the calibration-drift problem that limits every optical approach, and doing it with a cuff means putting an inflatable band in the strap — which other manufacturers have shipped, at a real cost in size and battery. Treat any specific claim about a future model as rumour until Apple says otherwise.
Is there an app that measures blood pressure on the Apple Watch?
No. Apps on the watch can log readings, remind you, chart them and read them from Health, but none can measure blood pressure, because the sensors to do it are not there. An app in any store that claims otherwise is not doing what it says.
Why does the feature need 30 days?
It is looking for a persistent pattern, not a moment. Blood pressure swings constantly with stress, sleep, salt and activity, so a short window would produce alarms from ordinary variation. The long window is what makes a notification meaningful.
